Here is the typical information requested in a tape tree form included in the signup announcement:

This information is used to determine who will be branch parents and who will be leaves, and where people will fit into the tree structure, so be certain to include all information. Also, try to do it in the same order as it was listed in the signup announcement because some tree administrators use a small computer program to organize signups into the tree structure automatically.
